LayerCreator sets phi binning inconveniently, consistency check of binning (was ACTS-359)

LayerCreator uses phi step size to enlarge the binning, but that appears to cause problems for the ATLAS pixel

Also: if surfaces are not filled into a bin, they are simply missing from the layer. this is a problem and LayerCreator should detect this
