De-organized shelf monitoring interface with more than one AMC

Summary

The shelf monitor web interface is messed up when trying to monitor more than one AMC in the same crate. Instead of having the "DAQ", "TTC", and "Trigger Rate" in the first row of each AMC tab, they are all displayed in the AMC tab. See the screenshots below.

Steps to reproduce

1. Start a xDAQ executive with more than 1 AMCMonitor in the same crate.

What is the expected correct behavior?

The first row (DAQ, TTC, and Trigger Rate) should be displayed in the correct AMC tab.
