Fix lhapdf output valerio

This is the third attempt to merge the lhapdf6 output into master: Fixes applied: -> src/lhpdf6_output.c -> src/init_theory.f are syncronised to smooth out the subgrid structure across thresholds using a "tiny" number. This commit is on behalf of Valerio.
