Pixel radiation damage fixes and optimisations

• fix non-thread-safe random number usage
• dereference read cond handles
• fix inconsistent whitespace
• add a few consts
• do not initialise what you do not need

The output is now reproducible.

I'm sorry for a bit messy MR but this is a result of debugging.
