Fixes coverity defects 19191, 19192 (uninitialised member variables). While there, also tidied up headers for these files. Corrected compiler warning seen in debug builds, mentioned in !62854 (comment 6726335) . Checked compilation in gcc11-opt, gcc11-dbg and clang16.
