Fix of pt smearing and pt cut; added efficiency parameterisation for LRT

Many changes for EFTRACK-160:

• still an occurrence of mismatch of pt units MeV/GeV: smeared tracks saved in GeV;
• corrected min pt cut from 4 to 1 GeV on output tracks
• more fix on monitoring for better debug
• fix the smearing of covariance matrix for smearing =0 (was putting all=0)
• as requested by LRT studies, one parameterisation of the efficiency is added which uses d0 as input parameter Tagging @anghosh
