Remove unused (and redundant) UTRawBankMap.h

Remove unnecessary inclusion of UTRawBankMap.h, and remove the header file itself. Note that the (unused) functionality defined in this header is a subset of what is defined in RawBank.h.

note: removal of this header requires that Lbcom!732 (merged) (which removes a redundant #include of it) is merged first, or at the same time.
