Fixes to monitoring histograms required for CalibMon

Renames erroneously named histogram in Hlt2DQ_DpToKmPipPip (cc @dkaminar @mstahl) and removes duplicate histograms in Hlt2TriggerEff* lines (cc @battista).

Required for (but not dependent upon) MooreOnline!376 (merged)
