This MR improves minor details in the meshing tools:

Update the READMEs, fixing #134 (closed)
Rename dfise_converter to mesh_converter
Install both binaries in the default installation directory bin/, along with the allpix executable
Improve the help texts slightly
Move mesh_plotter out of its "data" subdirectory
