Add flowchart to request an AQP

I might want feedback from @fdibello on this one: I'm proposing that we change the procedure to start an AQP slightly.

Right now we have a lot of back and forth on various channels before we create the jira ticket for the QP. I'm proposing that as soon as we agree that an idea is reasonable the institute create the ticket. We'll edit it later with the final description. I think this will make it all slightly easier for us to track.

The other minor change is that that the QP candidate should change the ticket status to "in progress" themself when they start. This ensures that they know how to interact with JIRA, and it also will automatically assign the ticket to them.
