Fix Bug in PV pointing constraint (primarily for VKalVrt analysis/BPHY11)

Correct bug in pointing constraint implementation. JIRA ticket ATLBPHYS-130.

Adam: This also addresses warnings from static analyzers and valgrind warning of undefined behaviour.

These changes have a large affect on anyone using a pointing constraint in VKalVrt analysis (which I think is only used in BPHY11).

This could also have a minor affect in other derivations but only to the extent of minor floating point differences.
