Significant AthenaMonitoring(Kernel) updates, MDT monitoring speedup

Improvements to core AthenaMonitoring + AthenaMonitoringKernel, and speedups to the MDT monitoring code, from profiling studies. (Speed improvement is ~ x5.) Also introduces cutmask option to reduce the number of fill() calls.
