Some small fixes

  • Added ASYNC_REG synthesis attribute to PulseSync.v which is required for Vivado
  • Add missing files to BSTDecoder.qip
  • Add BstDecoderPkg.vhd with SDB record for core.
  • Fix BST message URLs in BstDecoder.v
