Move the AMC13 monitoring inside the new monitoring architecture

Summary

After #140 (closed), the monitoring will be properly split: (1) the data retrieval and treatment is independent of the display, and (2) one xDAQ application exists per connection.

This is however not true for the AMC13 monitoring that would still be embedded into the AMC13Manager application. This issue aims at creating a new AMC13Monitor application, similar to the AMCMonitor one. Data can be displayed in a new tab in the ShelfMonitor Web interface.

What is the expected correct behavior?

The AMC13 is monitored similarly to the other boards.
