Add default GBT phases for GE2/1 module M4

Description

The file gemhardware/interface/gbt.h contains the default GBT phases only for the GE1/1 chambers long and short. This merge request adds the default phases for a GE2/1 M4 module. The default phases are obtained from a single phase scan on an M4 module for all VFATs except VFATs 3 and 6, for which a default value of 0 is used as the phase scan was unsuccessful.
