Fix indices between PA, ST and LU with correspondance tables

This commit consists of 3 changes

  1. Use IVCEP(9, i) for the integer vector
  2. Do not skip sk::MTRACK indices when looking up from a table other than VECP
  3. Use ISTSH and ISHST between ST and LU
