clean up unused functions

• Remove serviceAccountName and namespace functions that were never used
  • Fixes #111 (closed)
• Remove the following functions that are no longer used since !88 (merged)
  • utils.mgm_hostname
  • mgm.fullname
  • utils.mgm_fqdn
  • utils.mq_hostname
