[Muon decoding] Remove special treatement of first hit
Fix the following issues:
- Incorrect uncrossed flag for some hits
- Missing parenthesis causing a bug in TDC decoding
- Different lookup table for rec and allen, leading to differences in dx/dy
- Missing checks in rec-allen comparison tests
