Fix and improve monitor.py

• Use correct PhysFilter for 2016 (bit 95 for Hlt2 or any for Hlt1)
• Fix datetime conversion (local to utc)
• Fix directory location on nodes (for the unmirrored localdisk setup)
• Name output file based on stream (unless Hlt1 data)
• Handle keyboard interrupts gracefully
