Modernize PrLongLivedTracking

• add additional argument to Transformer instead of using an explicit DataObjectReadHandle
• prefer range-based for loops
• initialize member variables when they are declared
• move stand-alone struct into anonymous namespace
• fix one-time only memory leak by using std::unique_ptr
• avoid repeated push-back
• remove redundant inline qualifiers
