Cleanup output stream name definition and usage

While working on https://its.cern.ch/jira/browse/ATEAM-972 we noticed inconsistent usage of output stream name. In particular, it seems that during CA migration Output prefix was added to the name of the AthenaOutputStream. This MR tries to clean it up a bit.
