fix monitoringIDs in FTChannelID

Affects only FTTrackMonitor, FTClusterMonitor, and FTNZClusterMonitor

• Found & fixed bug in moniModuleIDStation that gives highest module ID=65 instead of expected ID 95 (up to 6 modules * 4 quarters * 4 layers)
• moniModuleID and moniQuarterID OK
• moniSIPMID needs further review (is there a problem in T3 where there are 6 modules?)
• moniChannelID needs fix, ID from mat should vary most slowly
